What the form collects
Our form asks who needs cover, whether you already have a policy, your year of birth, smoking status and cancer-cover preference. It also asks for your contact details and consent to pass your enquiry to a broker.
These answers describe your enquiry. They are not an assessment of which policy is suitable for you, a medical assessment or a guarantee of cover.

We introduce health insurance enquiries to Complete Insurance, an FCA-authorised broker. The broker may contact you using the details and consent you provided to discuss your requirements. Any quotation, explanation of policy terms or advice is the broker’s responsibility.
Does submitting the form start insurance?
No. A form submission is an enquiry only. Cover is not in place because you completed our form. Whether a policy is offered and when it starts are matters for the broker and insurer. There is no obligation to proceed.
